DIURETICSDefinition:

1.        Systolic > 140 mm Hg
2.        And/or Diastolic > 90 mm Hg
The blood pressure must be measured in a sitting position after a resting period.
Readings should be on two occasions, a pattern is more .
important than single readings
In 95% of cases no cause can be found; renovascular hypertension
should be excluded.
Diabetics or CCF
Systolic > 130 mm Hg
And/or Diastolic > 85 mm Hg

Step 1:
Life style

·            Reduce alcohol intake
·            Improve risk factors (lipid levels, smoking,  and blood sugar levels if diabetic)
·            Restrict sodium intake (< 2g daily)
·            Exercise and optimal weight

Step 2:
Start with monotherapy and lifestyle changes.
Uncomplicated hypertension Start with diuretic
Choice of drugs : keep age, race and concomitant conditions
and target organ involvement in mind.

Monotherapy
1) Diureetics
2)Beta-blockers
3)ACE inhibitor
4)Angiotensin receptor bolcker. (Only if ace inhibitor not tolerated)
5)Calcium channel blocker
Combinations
Diuretic and Beta blocker
Diuretic and ACE inhibitor
ACE inhibitor and Calcium channel blockers

SPECIAL CASES

Pregnancy induced hypertension:
Methyl dopa 250mg – 500mg b.i.d.
Diabetic
Type 1 diabetes with proteinuria
ACE inhibitor in combination with diuretic
Cardiac failure
ACE inhibitor plus diuretic if volume overload
Betablocker
Diuretic: Furosemide or Spironolactine
Angina
Betablocker and Calcium channel blocker

LISINOPRIL WILL ONLY BE CONSIDERED AS SECOND LINE ACE WHERE A HIGH DOSE ACE IS INDICATED
